We would like to thank you for choosing Ware Family Dental as your new Family Dentist!
FIRST VISIT
Upon calling to schedule your first appointment, we will ask for your insurance information, date of birth, and social security number. By obtaining this information prior to your appointment, we can serve you more efficiently upon your first office visit.
As a new patient, you should bring your driver’s license and insurance card(s). We accept all insurances including CHIP and Medicaid. Also as a new patient, child or adult, we will take all new x-rays if necessary so you do not have to bring prior medical records (unless you would like to).

The forms below are for you to download, print, and fill out.
Filling this preliminary forms will help save you time on your visit to our office. However, you are more than welcome to wait until your appointment to fill the New Patient Form out.
Insurance
We accept all major insurances, including Medicaid and CHIP. Upon scheduling your first appointment, we will ask you for your insurance information, including name, social security number and date of birth. We do this as a convenience to our patients to ensure less time in the office by having the insurance portion of your new file complete prior to the time of your appointment.
Cancellation
If for some reason you can't make an appointment, we kindly ask that you provide us with at least a 24 hour notice. This is so that we can offer this appointment to another patient in need of dental care. A fee may be charged if sufficient notice is not given.
Financial Policies
We will gladly submit all insurance forms and help you maximize your insurance benefits pertaining to your dental care.
We accept all insurance, including Medicaid and CHIP. Payment is due at the time service is rendered. Forms of payment accepted are Visa, Mastercard, check, cash, or debit. Also feel free to enquire about our Care Credit payment plan.
Facility & Operating Hours
Our Grenada, MS office is conveniently located less than a mile off of Interstate 55. We have a kid-friendly waiting room, and a staff that is ready to help you anytime.
